The Community Outreach Team is comprised of RNs, LPNs, Health Educators, Social Workers, and technical and support staff who are committed to providing services that will improve the overall health, medical knowledge and safety of our patients, family members and the community at large.

Our services are provided on both the inpatient and outpatient basis.  You may connect with one of our staff members while you are a patient at the hospital, or be referred as an outpatient by your physician.  Either way, we are with you for the long road, providing you with the care and assistance you need to live a safer, healthier life.
