Jacob Tallman MD, a board certified family practitioner with a fellowship in geriatrics, operates Creekside Family Medicine at 12 North Church Street, Canaseraga, NY. Dr. Tallman grew up in Nunda, NY and has returned to the area to work with Noyes Health and run his outpatient practice. Dr. Tallman received his undergraduate degree from SUNY Geneseo and he attended Medical School at SUNY Buffalo. Dr. Tallman runs a successful practice and is currently accepting new patients.
